Pet care waiver

Pet Care Liability Waiver & Agreement. This agreement covers everyday pet care (walks, in-home day care, nail trims, medication administration, and related care) and is separate from any wedding-day services agreement.

Health & vaccinations. My dog is in good health and current on rabies and core vaccinations, except as noted above. I will disclose any illness, injury, or change in behavior before each visit.
Behavior. I have honestly described my dog's temperament. I am responsible for my dog's behavior and for any injury or damage it causes to people, animals, or property.
Veterinary care. If my dog appears ill or injured, I authorize Tail & Veil to seek veterinary care at my expense and to make reasonable decisions if I cannot be reached.
Assumption of risk. I understand that walking, day care, and handling carry inherent risks, and I assume those risks. I release Tail & Veil from liability for injury, illness, loss, or escape not caused by its gross negligence.
Home access. If I provide access to my home, Tail & Veil will use it only for the agreed services and keep it secure.
